Penis numbness from years of pressure on perineum

Hi,

I hope someone could give me advice. I wear a prosthesis and long story short, the prosthesis has been putting pressure on my perineum and right where the buttocks meets the leg, near the butt bone since 2007 to present day.

My current symptoms are difficulty passing urine, lessened sensation on different parts of the penis as well as less intense orgasmic sensations. During orgasm, I would feel like the sensations move from the scrotum to the middle of the penis, but not the head or where the circumcision scar is, like its supposed to. Right when I'm about to ejaculate the sensations are there, partially, but when the contractions to expel the seme begin everything goes numb.  I also have sporadic symptoms of a cold, flaccid penis.

I've already seen a urologist. They did a urine culture and physical examination and say everything "looks" normal. No evidence of infection. I do suffer from low total T, but my free Testosterone is normal range. Erections are not as hard and long-lasting as well. It generally feels like the numbness is caused by some misplacement or swelling of the prostate. I asked for imaging, but they said since I dont have a history of prostate cancer and I'm under 65 y/o, my insurance wont allow it.

What kind of urologist or neurologist specializes in nerve injuries and/or prostate issues. My mom says she doesnt remember if someone in my family has had prostate cancer, but my issue seems injury related. Any advice would be helpful.
