Hi,
I have used self catheterization since about 1996 and on a more frequent basis since 2003.
With the self cath and suprapubic currently, I'm still coming down with infections. Just had a urine culture done today and have a low grade infection.
I used to come down with UTI, kidney infections, epididymitis and orchitis about every 3 to 4 months requiring a week long hospital stay. In 2008 while being hospitalize with pneumonia and a kidney infection I discovered that I was passing gas through the penis and catheter. I was referred to a urologist and after a cystoscopy it was confirmed that I had a fistula between my prostate and rectum.
Prior to this I had undergone a TURP and 6 o'clock resection of the bladderneck in 1995.
It is possible that the area between the prostate and rectum was weakened during this procedure and therfore caused numerous infections. Urologist thinks that it was caused by self catheterization. I don't think so.
Before treating UTI it is important that they find a cause for the infections. Having a foley catheter in can be a source of the infections.
While I was hospitalized in 2000 for an emergency left orchiectomy and kidney infections, they treated the infections with Flagyl. This drug was chosen due to the fact that some organisms can live without oxygen. I had a staph infection in the left testicle as well as double kidney infections. It took me several months to recover.
With your brother's TBI it is most likely that he has little to no control over his bladder function. I have simmilar problems due to neurological problems and I was diagnosed with a neurogenic bladder. In the case of your brother there might be urine retension if a catheter is not used.
As smfirkins05 commented as well as I have stated some organisms don't require oxygen. stenotrophomonas maltophilia. In my case the Flagyl got things under control, however with my neurological condition, Flagyl is harmful to my neurvous system, but there was no option, the infection had to be controled.
Just last September I came down with an abscess in the right inguinal area as a complication of my suprapubic catheter. They once again used Flagyl to control the infection. As a result I have more numbness in my hands feet and legs.
If your brother had no underlying medical conditions prior to the accident he should not have any problems with the Flagyl. One side effect is that it gives a very bad metalic taste. It is a wonderful diet control. With my incedent in 2000, with in a month I went from 5' 6" 140 lbs. To only 95 lbs. when I was discharged.
Have the doctors do a urine culture to determine what they are dealing with. Only that can the determine proper treatment.
All the best.
Ron
