UTI with Self Catheterization

I am writing about my wife, aged 64.  She had TB form her infant-hood to the Right Kidney that was discovered after a checkkered treatment from her 23rd year for troubles from this underlying cause in her 31st year.  Proper treatment was started and the case trasferred to newly available Urologist who were rare then.  TB treatment went on for another five years and in the middle she was operated and that kidney was removed.  Concurrently she had UTI and that was so severe with incontinence pus and urine used to fall on floor while moving. When this trouble came under control the opposite started namely urinary retention.  Urine dribilling only when bladder is full. While treating for this she was taught self-catheterisation.   Since much greater trouble of badly treated heart-attack though it has within hospital equipped for that and resulted in necrosis of diaphram and severe reduction in blood pumping, catheter subject gone out of attention.   At the end of second attack she got into cardiac arrhythmias.  Now she has only subnormal temparature may due to heavy doses of paracetamal that were given some local viral fever. So it has become difficult to guess whether she has fever with uti and uti is to identified from urine examination that is not practicable.  As a preventive measure our Urologist suggested to send-in after emptying urine, Betadine solution diluted to half, through same catheter.  Since that was not flowing in, I cut a foleys catheter at the end and connected syrange to pump in 3cc of this liquid and drain out with other cather.  Are we in the correct and safe line?   Prior to this with uti the creatinine wentup to 1.9 from stable 1.6 % .

I thank you for your kindness to respond and with concern.
The catheters are Foley's 14size.  They are never used simultaneouly.  After draining urine with regular one and since betadine solution as advised by our Urologist is not flowing in through same port of catheter urine has come out even if raised to possible extent, except the solution being wasted,  I devised another catheter with injection syringe (without needle, with its bare nozzle, onto which the cut end of the catheter slipped on) and asked her to remove catheter with wich urnine is drained.  Then to draw upto 3ml solution into the syringe through inserstion end of catheter and hold stright up the assembly so that surely if any air entered in would be expelled by pressing syringe until a little solution comes out and then insert and push in to usual depth and then close syringe at comfortable pace until all solution gets in and then remove this one.  After a minute or two use original catheter  and completely drainout bladder contents.  The betadine pumping exercise repeated twice a day.  As for using new catheter each time is beynod means since catheters are costly.   This our urologist did not since 20 years and in the I my self was skeptical about this fearing multiplication of bacteria and doctor reassured us and got frequently checked and all was well and we persued this proceedure.  Infection is noticed recently only, her Cardiologist reffered to Nephrologist the pus was noticed and when that doctor said in our second appointment that uti is common among selfcatherizing patatients, we got alarmed until she slides in to dialisys stage she may not full attention, and ran to the Urologist who taught this technique of urine voiding.   There after medication as the infection was persisting Cystoscopy was done and bladder washed in January this year several years after and this was very common in seventies for some years one or times a week perhaps for cleanin bladder and to stimulate sphincters to provide her control.  In the internet on one of the sites it is said the golden rule is to drian until the last dopp is drained each time. since it is retained urine that give rise to bactiria.
Since  the UTI  haunts these kind of paatiients and since geometry of ladies catheterisation is stright and simple but for the insertion is confusing and to recurrent infection I want to get  a silver one for her not only silver intrinsically antibacterial but also canbe sterilised easily.  Our urologist also agreed and wanted to help but the agent whom he hoped to get us a plastic model was not turning out.  Can we get help here for design of it for patients using 14 Fr Cath?
Onece again I thank you, for your recognising of our trouble and my endavour to take care of her, which she herself realises little.  The non medical part is that in our religion, the first girl whom we marry becomes religiously important so much that without her no important religious duty is possible. Our marriages are not for enjoyment and convenience but for religious puorpse and it is binding here and hereafter for both.  I need to do some religious work that I am not able complete sofar and until we complete it I should carry her into future with all my energies.  Say since even before my youth waned it was evident that she cannot conceive, to marry one more leagally is easy, but religiousy her approval will be mandatory and she will be unrewitten manager of house.  Can there be any other society with full freedom ladies more secure than here?   Just for reflection. Of course this system got damaged with penitration of other cultures.

Hi,

I understand this has been hard for both of you.I just want to commend you for being strong for your wife.

With regards to infusing the betadine solution,have you informed your physician on how you do this? As you have posted,you used two catheters ( is this correct?). I still suggest that you discuss your concerns with your wife's attending physician so you may be properly guided. Observe sterile techniques at all times when doing this .The reason that it is best to use the same catheter in infusing the solution is to avoid contaminating the otherwise sterile catheter that is in place.Once the catheter has been removed,it is not advisable to use it again.

If there are any questions with regards to this always  inform your physician.
