What caused this scar on my penis?

Here's the story.  When I was 11 years old, I received a second circumcision.  It was really for no reason, as almost every circumcision is; I only went through with it to get a day off from school and I didn't know any better.  Anyway, I left the hospital the same day and the skin was as tight as a drum.  That night, while I was laying in bed, unable to sleep due to the immense stomach pain caused by a Tylenol with codeine that I had taken earlier, I got an erection.  I flexed my kegel muscles to see just how tight I could make my erection, and suddenly I felt something pop or snap and instantly I became flaccid gain.  I went to the bathroom and there was a giant scar stretching the entirety of the underside of my penis.  This scar is still there today and it is very noticeable.  I never told anyone because I was embarrassed.  I have since started a manual foreskin restoration regimen (tensioning the skin with my hands to induce mitosis) and the underside of my penis hurts disproportionately when I am tensioning.  Does anyone have any idea from this description what happened that night and why this scar formed?  Also, is there any danger in applying tension to the area the scar is in?
