enlarged cord pain and changed chape of scrotum,

my left  testis was removed 10 yrs ago.

Four year ago i was hving pain in my rt testis and then after in doppler and usg of whole pelv and abdomen nothing appear except in a report in which grade1 varicocele appeared and i was undergone a varicocele surgery as well rt testis fixation and cystoscvopy at the same time.

Now again the same thing is happening, in my scrotum a vain comes out apparently at the tip of testes, epididymis is also paining. I show to doctor he said its like epidymo orchitis, i m taking cefixime & levofloxacin medicine since 20 days but no cure is showing. And the cord is geting bigger in my scrotum and pain is also thr. Can anyone tel me reason that why that cord which connect epidydimis is getting bigger like baloon (No harnia as USG scrotum si done - it showd all is well, just edematous spermatic cord is thr) and the shape of scrotum also changes as biger. CAN ANY RESPECTED DOCTOR OR FREND TELL ME Y THIS HAPENS AND HOW MUCH TIME IT TAKES TO CURE???? M SO MUCH FRUSTRATED AND UNCOMFORTABLE.

Further, if i do transilumination at home, where shall I point a torch and how it will look if everything is normal?? I know after reading this you will say me duffer but as i dont know the process so i am asking. And is spermatocele curable??? is it infection or wat?

Hello,
As mentioned earlier, it can be a speramtocele or epididymal cyst. Often a spermatocele does not cause symptoms. Symptoms, when present, can include pain, swelling, or redness of the scrotum or a feeling of pressure at the base of the penis.

You can get done a transillumination test or an ultrasound. Spermatoceles are filled with fluid, so light will shine through them (transillumination). Light will not pass through solid masses that may be caused by other problems, such as cancer of the testicle.

1st of all thank you very much for kind reply.

I did ultrasound, it showed only edematous spermatic cord, and tht ultrasound doctor said, may be infection., but what is that cord like thing which comes up at tip of testis make the scrotum look bigger and disappears again.  when testicle is loose and rest position it feels totaly normal and again after some time tht vein come makes scrotum biger, the skin of scrotum has got thick and ache is there. Kindly understand the matter, no discharge from penis or no itching, just pain and tht cord and thick skin.

Hello,
These symptoms can be due to epidiymitis, swollen epididymis, cyst in epididymis or varicocele. The vein that you are feeling can be due to swollen and infected epididymis (a narrow, tightly-coiled tube on top of a testicle) and the testicular pain can be due to epididymitis. This is more common in young men and may cause a heavy sensation in the testicle area. The area becomes more and more sensitive to pressure.

You need a physical examination from an urologist along with urine analysis, complete blood count, testicular ultrasound and scan and tests for chlamydia and gonorrhea. Treatment will depend on the cause.
