Left Abdomen and Left Testicle pain with 45 degree angle to urine stream?

My problems started in 2004.  At that time I didn't really see anyone about it cause I had just had an appendectomy and thought it could be an issue with that.  At that time, the FIRST time I ejaculated, it was this completely clear jelly like substance.  I thought maybe it was due to the cathedar they had put in me for two days while in the hospital.

Anyway, about 2006 I started seeing urologists about it.  They said it was epididymitis.  I took antibiotics and it seemed to go away.  But soon i realized that this was normal--the pain would come and go on its own.  

Around 2008, I went to another urologist, they did an ultrasound--nothing abnormal found.  Once again they chaulked it up to an infection.  They gave me antibiotics, I took them pain went away on its own about two weeks later.

All during this time my ejaculate never went back to being white again, and I now I have episodes about twice a month.

In 2010, I had another ultrasound done when I switched primary care doctors.  It showed a veriocele but otherwise normal.  He suggested I had non-bacterial Prostatitis, since all the urine tests never showed infection.  Before this i would show high elevated white blood cells, but believe this was due to chronic sinus infections that I had corrected with surgery.  After that surgery, my WBC went back to normal levels.

Now new symptoms have appeared.  I get low back pain with lower left groin pain and the pain in my testicle is always there.  I have notice my testicle has shrunk and is hard.  comparison between the two ultrasounds showed a .5 cm decrease in size.  My ejaculate is completely clear and runny like water.  In addition, when I urinate my stream veers to the left a good 45 degrees.  Symptoms seem worse after a large meal and with heavy lifting or excessive activity.  I am not overweight.

Doctor say everying is fine urologically.  Any idea?  I just made an appointment to see a shrink cause I think i'm going nuts or something.

Did you test for STDS ? Herpes ?

Urinating at 45 degree angle can be caused by an enlarged prostate, or as in my case, a hernia pressing against the internal portion or the penis.  My ejaculate differs and become more clear if I have sex a long time before I ejaculate.  I wouldn't worry so much about the ejaculate, but I would not ignore your pain.  See another doctor and simply tell him about the pain you feel and the 45 degree angled urine.
