My 16yr old son for nearly 4 wks has been having chronic testicular pain that is increasing in severity to where stronger pain meds are needed to relieve the pain ie T3's to morphine shots. He has had an ultrasound done on both testis, kidneys, an bladder with normal results he also had normal results on a CT scan nothing abnormal had been found so pretty much all "common" diagnosis of testicle pain have been ruled out, cancer torsion orchritis.... So where do we go from here? He is such severe pain but what's causing it is a "medical mystery". Are there any further tests that I can request to maybe help pin point the problem so that long term treatment can be sought rather than continual pain treatment.
I have had blood tests and urine tests completed. Both came back with no findings. If it was a kidney or urethral stone, wouldn't it be painful upon urination? Shouldn't the xrays and ultrasounds have picked up a testicular tumor? Isn't testicular cancer painless? The doctor had also mentioned I had a hydrocele but I believe those are painless.

As there is no swelling or inflammation or redness of the scrotal sacs, so the chances of epidiymitis or orchitis are rare.pain due to torsion comes suddenly and is severe and acute in nature. Moreover it is often accompanied by nausea and vomiting. Pain in varicocele subsides on lying down. So in your case it can be a hernia or a testicular tumor or kidney or ureteric stone.
The final diagnosis can only be made after a thorough physical examination including examination of the abdomen, scrotum, testicles and rectum.
Lab investigations like complete blood examination, urine examination, ultrasound lower abdomen and sometimes a urethral swab will help in diagnosis. Treatment will be started thereafter.
