bleeding after masturbation and excercise. Could that be related to PCOS?

Hi i will try to explain my question as much as i can and please excuse my English.
i'm a 16 year old female, i have been bleeding after masturbation and exercise but i can't recall since when it started happening, i had my first period when i was 9 years old and i have never had a regular menstrual cycle i would stay 3-5-8 or more without a period and when i was 15 i started bleeding lightly (or spotting) brownish blood and sometimes red blood, i don't remember for how much time i bled but it was a lot and at first i thought it was a period but it didn't stop, and i remember it usually starts after i masturbate and goes on for days. I didn't think much of it, after a while i decided to go to a gynecologist to find out why my periods aren't regular and i was diagnosed with PCOS, i think (not sure) that i told her about the bleeding but she didn't comment on it, she prescribed primolut (Norethisterone) and Dian-35 for me for 3 months, primolut to induce a period first and then i take dian-35 on the fifth day on my period
My doctor also advised me to lose weight, I used the prescription (note: as soon i started primolut the bleeding stopped) and started dieting for 3 months, after 3 months i stopped using the pills and waited for my period to come but it didn't, so i went again to the gynecologist but she was on vacation so they referred me to another doctor and she prescribed me dian-35 and glucophage for 6 months, she also said i have very weak ovaries and i have cysts on both ovaries, I started using the prescription again and dieting for 6 months, i kind of failed at losing weight and gained all the weight back, after 6 months i stopped the pills ( until now i had 9 cycles on the pills) and i had a period on my own after that about 4-5 months later
After all this my period never came back and i didn't want to go to the doctor again because i wanted to regain my period back through diet (which i fail every time at).
So after about 6 months the bleeding happened and again i thought it was a period but it kept going on for days and then months, the bleeding usually starts after i masturbate (with penetration or without) and after i exercise (weight lifting, cardio, HIIT) and it seems to stop after 1-2-3 days, but it happens again because i exercised or masturbated, and sometimes it just happens and stays for days, the bleeding is reddish brownish color mixed with stretchy discharge, and it looks more red and bright after exercise and masturbation. so after 4 months of bleeding on and off i decided to take primolut to induce a period AND to stop the bleeding (because it stopped the bleeding last time) And it actually worked and the bleeding stopped and i had a period a few days later.
I am so sorry if i confused you with the timeline or my bad English but i am trying to be specific and include as many details as i can.
So now it's been 4 months since my last period and the bleeding had started again about a month ago,I am sure it is not an infection or something I'm very hygienic and i only use my fingers when i masturbate, and i'm also very gentle so i'm not cutting myself or something.
I am so confused, why is this happening?? Why do i bleed after masturbation and exercisee?? Is it related to PCOS? Is it entirely another problem?
Could it be something serious? Why does the bleeding stop when i take primolut?
I don't want to go the doctor again as the topic of PCOS will come up and i will be put on pills again.
I'm sorry again if i confused you and i hope i get some help from you guys.

Do you have pain at other times?  I'm wondering about endometriosis?
