intense itching..scrotum and penis

I am 24 yr old male.. I have been suffering from intense itching on sides of my scrotum and also on the upper side of penis..i think it slowly developed over two years at first I had itching on the scrotum over limited area and it was more intense at night.. Then it slowly involved whole of the scrotum and now penis also.. Itching is very intense at night and also results in cuts and wounds and skin becomes very dry and ultimately sloughs off then new skin grows and it feels better but after sometime the cycle repeats and it is spreading.. I have applied antiscabies lotion and also taken ivermectin but there was no improvement and also the itching is limited to my genitals only which I think in scabies there is not such a limitation and whole body can be involved.. After that I never consulted any skin specialist but my problem Is getting worse.. I also see today palpable lymph nodes in my groin.. What should I do?? Is it scabies or what can you please make a diagnosis?? Waiting anxiously.. :(

It sounds like yeast (jock itch), but you have let it go a long time.  

It is also possible to get psoriasis there.  You should see a urologist and find out what's going on.  I do not think dermatologists are usually much help.  Particularly I do not like the steroid creams that dermatologists love so much.  Many guys have complained of the skin thinning there, and it doesn't come back.  So try Lotrimin cream as directed on the tube, and if it helps, finish out a two-week course.  You should throw out all your underwear or at least wash them in hot water with baking soda to kill the yeast.  Do your socks in too in case you have the same fungus of the feet (athlete's foot).  People pass it back and forth between these sites.
