As these symptoms are continuing to persist it is definitely time to contact your physician. Give him a call. Don't drink anymore coffee for now.

Psychotherapy, cognitive therapy and behavioral therapy are not always enough on there own, but drugs never are enough. They are a bandaid that doesn't change or fix anything. Psych meds need to be paired with some sort of therapy. Please either go see your doctor immediately or got to the ER if you can't get right in with your doctor. There are quite a few things that can cause SOB and chest discomfort and most constitute an emergency. Good luck to you. Please keep us updated.

I've had a LOT of psychotherapy in the past, and it doesn't help but for some reason people think it does? I guess it just doesn't work for everyone.

I have tried therapy but it doesn't help.
I am still feeling tighntess and discomfort right now, and it has been almost 6 hours since I've had 5 mg of methyphenidate. Total methyphenidate today has been 20mg. My script is for 60. I have tried amphetamine salts too, that seems to have fewer side effects. My plan is to switch back to that.
You should contact your prescribing physician about your symptoms because SOB and chest tightness shouldn't be ignored. " Excessive amounts " of caffeine isn't a good idea either! Its dangerous...

Heart palpitations are definitely a side effect and so is increased anxiety.
Caffeine will certainly increase those side effects. Do you also counsel with a therapist?
Do you have normal blood pressure?
You take 60 mgs three x a day?

I haven't spoken to my main psychiatrist about the issue. I'm afraid he'll never give me a stimulant again.
Before methylphenidate, I never got out of bed and when I did I was sleeping on a couch. With the methyphenidate I've started doing software engineering again and starting to get my life back. I'm not going to risk loosing the only drug that has helped me, but I wish there was something safer to take.
I have been taking 60mg 3 times a day for about 1.5 years i believe, and have never had a problem before.
This summer I *DID* take more than 60 for about a month (don't know what I was thinking), but dropped back down to 60/day and have been on 60 again for a least a month now, but for the last several days I've taken much, much less because I'm having chest tightness and trouble breathing.
I went to ER and they said it wasn't a heart attack. I think excessive amounts of caffeine may also be a factor.
