Third party speaking occurs with pervasive developmental disorders (autism spectrum) very often....many times children who are diagnosed with PDD's earlier in life turn out to be bipolar........Beware of "bipolar" diagnosis in young children - good pshych will not diagnose a young child with bipolar....I would say  - speaking in 3rd party is not that unusual for a 3 year old, or even a 4 year old, but it should be fading with time.....

How are his reciprocal communication skills?

I know it is frustrating. Make sure that he does not see your frustration - he will pick up on it and it will likely make things worse. I knew actually an adult that reffered to himself in a third person
Speech therapy adresses these issues

Model appropriate ways - he says "**** loves mommy" - say "I love mommy", etc.

Do you speak that way to him?    I'm really surprised at how most kids don't speak in the third person,  although their parents do.  "Look at Mommy"  "Would Brandon like a cookie" "Give Grandma a hug".
