HELP. I'm desperate

I dont really know if my problem fits in the GERD category, but Im desperately looking for a solution to a problem that has been destroying my social, professional, romantic life and self steem for decades: a killer bad breath. This week I had the most embarassing experience, which brought me down to my knees. I decided to make the most of if and fight HARDER for a solution and I hope you guys can help me.

ISSUE:
- Constant room filling killer bad breath;
- If I drink alcohol I will burp a smell that resembles feces. It infestates the entire room. It's hedious! Some friends already noticed that it only happens when im in the room. It's like a bad bad bad bad f...rt and I think it comes out when I'm talking.
- It seems my breath can be projected many feet away. People on the other side of the room will cover their noses.
- I've dramatically improved my oral hygene and 'I DONT THINK' the smell comes from my mouth. People will still cover their noses right after ive done a thorough mouth wash;
- In the past I had tonsilnites which made my breath even worse. My tonsils were removed 20years ago but the problem persisted.
- I think i have acid reflux which caused gastrites in the past. I also remember waking up breathless as a teenager. I dont get so much heart burn nowadays cause i cut off fats, acids and alcohol from my diet,
- I drink a lot of protein shakes as I workout hard, but it only affected the smell of my feses and gave me bad smelling gases which I can hold, so it doesnt seem to be an issue.

MY HABITS
- I shower 2-3 times a day
- I keep my bed inclined by lifting it with a pile of 2 phone books (10cm)
- I brush my teeth, scrap my tongue, floss, gargle baking soda mouth wash 5 times a day
- I eat very healthy (fruits, fibers, lean home made dishes, etc). I also drink a lot of protein shakes;
- In terms of diets i do like sweets and chocolates and i eat it frequently. I was tested for diabetes and its negative.
- I work out and do sports daily. My body is ripped, so i feel very healthy.
- I dont smoke or drink (unless i wanna burp foul gases)
- I've been washing my nose with salt water using a Netty pot.
- I take vitamin suplements: B complex, Zinc, acidophilus
- I started taking digestive enzimes 3 days ago.

MY HEALTH
- I do medical checkups every year and all seems ok.
- I do have allergies. Had sinuses inflamation in the past.
- I do feel a lot of mucus on my nose and throat. Im now scraping my tongue deeper down and feel my throat less congested.
- Overall Im really healthy and look 10-15 years younger than my age (Im 43).
- I did have bad teeth as a child and teenager and have a lot of fillings and crowns, but i do dental checks every 6 months and there's no apparent issue there,
- As a child, I had my first teeth 'burned' with silver nitrate. Not sure if that affected my organism at all. I think not.

Please let me know what direction I should go. My doctor doesnt seem to care.

Well, after the long line of suggestions I wonder if it's a simpler one.  My husband and I both have GERD.  We both had bad breath and were healthy orally.   Not sure if you eat these, but cutting out all onions and garlic including garlic pills and spicey foods did the trick.  The garlic and onions were the biggest cuprits.  No spices....none.....just salt and pepper.  Also, tums weren't helping either. Good Luck

After reading some of the concerned regarding GERD and it's relationship with bad breath, needless to say it makes me feel better in a way...knowing that I'm not alone trying to find a solution with my bad breath issue. In my case, I am in an "on and off" state! However, this time... I guess it's time for me to keep moving forward.

Based on my extensive online research regarding the possible caused of  my bad breath problem. I got to the point where my doctor prescribed  a medication for threating Helicobacter pylori bacteria(something for stomach). it's a combination of medication in one box. After 2 to 3 days after taking the medication together without soda,coffee, and other food or drinks that I thought that causes my problem, I felt that my bad breath (fart like smell or sometimes worse) went away. I can tell right away when I tried to smell my breath  and by trying to talk in front of the people to see if they will rub off their nose. that was long time ago, I liked that feeling and I missed that feeling... my badbreath due to my stomach acid or GERD is back. it came back due to my temptation to coffee, and alcohol and other factors.... where ever I go right now, I know that when ever I talk to someone else they can smell it, worse part about it is that it's coming from my stomach, and another worse part about my case is that I work in an office dept. where there is not much air circulation... I just realized lately that the reason why they keep the door open is to let the air out (my odor) and in addition to that, anyone of you who's suffering like I do (you know how the social issue goes) so I'm going to see my doctor again and ask for the same medication avoid that stuff that my cause GERD and hopefully if it the way it should be!!!

Hello all. I want to say first of all, the internet has brought us all together and given us the opportunity to help each other. What a blessing.

I started having bad breath when I was 14. I am now 35. I went through a lot of the stress and confusion that many people are describing in their posts. I found a breath clinic where I live about 10 years ago. Prior to that I suffered through one embarassing moment after another. Prior to the advent of the internet, all i had available to me was dentists and doctors. I made the most of them. I discovered i had tonsoliliths. Those things smelt awful. I had my tonsils removed in my twenties thinking it would solve the problem. It probably helped, but it did not cure the problem.

I went to the dentist, he could not help me. My family doc sent me to a gastroentorologist about 18 years ago. He diagnosed a hiatal hernia and told me that the eosophagus is a collapsed tube and therefore the hiatal hernia could not be the cause of my breath problem. It was good of my GP to send me there though, it showed some good insight. I left the hiatal hernia alone and filed it away in 'not causing my halitosis'.

Eventuall the internet came around and this led to me finding a dentist who had a halimeter in a major city. I went there. He referred me to a new breath clinic specializing in oral malodor. Those people have helped me a lot. Saved my life I think. They used to have a halimeter. In the last few years they switched to using the BANA test (saliva) and the Oral Chroma as diagnostic tools. The Oral Chroma is a gas chromatograph that focuses on the 3 known major VSC offenders in bad breath. They are Hydrogen Sulfide, Methyl Mercaptan and Dimethyl Sulfide.

About a year ago something really interesting happened. As usual, I get put on an oral antibiotic to get me back down to baseline when my usual rinse and protocols seem to be losing the battle. I take 2 weeks of someting like Flagyl or another oral antibiotic and I get back to zeror and then get on my rinse which is a chlorhexidine base rinse.

The Oral Chroma was re-administered after the administration of the antibiotic. My clinic never used to have the follow up right after the antibiotic was used. I used to follow up only 6 months later. What was revealed in this new 'earlier apt post antibiotic' was shocking. The Methyl Mercaptan an the Hydrogen Sulfide was gone entirely. We also can confirm that nothing was coming from my tonge or oral cavity. The Dimethy Sulfide however experienced no change at all. This should not be after a 2 week course of antibiotic where you rinse and gargle your throat.

This had my dentist at the oral malodor clinic perplexed and he told me that I needed to go see and ENT and a gastro via a referral from my family physician. Prior to taking that step I decided to comb the internet some more to help myself prior to going further.

I can tell you that I often had the strange experience described by some of the people on this forum whereby somebody in a group of people will say 'who farted'. I can never smell it. Once I was in a room with my friend's dog and I was talking and playing for a while and my friend comes and says 'did you fart'. Again I cannot smell it. I think perhaps it was the dog and I missed it. Then it keeps happening again and again, this time there is not dog, it is only me and one other person in a room and I cannot smell it and people start pointing out it is my breath. I was on a basketball team at one point and it seemed that people all the way down the bench could smell that fart smell. It literally drove me crazy and still does to this day.

Fast forward to a few months ago I am looking on the internet. I discover that GERD can have no symptoms. That the reflux need not cause heartburn. I do get heartburn sometimes though. I find out that GERD can lead to irritation of your throat and that your sinuses end up generating a lot of mucus to coat the throat and that this mucus can be an special stickier kind. That mucus can become fuel for the production of VSCs and cause a major problem.

In the middle of all of this I get diagnosed with sleep apnea. You may be saying why is that relevant. I will tell you. I wear a mask with my cpap machine which flows continuous air pressure through my nose only to keep my airway open when I sleep. I have notices at times that when it is blowing and I swallow and then exhale out my nose in to the mask right after swallowing that I detect a very foul concentrated fecal fart like odour. I ask myself what the heck is that. It is like somethow the mask is altering my olfactory abitlity and letting me smell a concentrated version of what is in my throat. I file this away into the interesting category as well. I start to thing perhaps the GERD is involved and that there is mucous in my throat and what I smelling is some VSCs being released from the throat mucoud situation into my mask.

In my research on the intermet I came across an astonishing paper publised online by a Dr. A. Tangerman. He and colleagues appear to have notices in their paper a new kind of dimethyl sulfide fuled 'hitherto unknown blood borne halitosis'. I wondered what he meant so I dug further. After all, I am dealing with Dimethy sulfide myself.

I discovered that a genetic metabolic condition exists whereby a person' breath sweath and/or urine in any combination can smell like boiled cabbage. Some people have serious neurological symptoms with this disorder, but many do not have any other symptoms other than smelling. I know from research that boiled cabbages smell is often associated with dimethyl sulfide. You breath can smell that way. The name of this condition is hypermethionemia. It goes by a number of other names such as:

    * Deficiency of methionine adenosyltransferase
    * glycine N-methyltransferase deficiency
    * GNMT deficiency
    * Hepatic methionine adenosyltransferase deficiency
    * MAT deficiency
    * MET
    * methionine adenosyltransferase deficiency
    * methioninemia
    * S-adenosylhomocysteine hydrolase deficiency

I thought to myself that this find was striking because Dimethyl sulfide (DMS) is also known as  methylthiomethane and this disorder actually causes your blood to pile up methionine or something in your blood and the by-product is dimethyl sulfide coming out your breath.

Dimethyl sulfide has a characteristic cabbage-like smell that becomes highly disagreeble at higher concentrations. I won't post all the research i found here, but this condition exists and I might be suffering from it.

As it stands I need to figure out if I have either:

1. Hypermethioninemia
2. GERD either causing or contributing to my dimethyl sulfide problem due to reflux and mucous combo.

I don't know how to get tested for number one or how to figure out number two let alone treat either one of them. I have theoris involving diet manipulation etc.. in any event I thought I would throw this out there to collaborate.

Something strange is afoot involving dimethyl sulfide.

ok, that's what I figured just wanted to be sure! My reflux is usually silent so I never really feel it coming in my throat. But the BB is there so I know that I do reflux even if I don't feel it. I said I was going to walk after dinner too but like you the drive is not there. Plus it's getting cold in my area. I will try out the 3 hour rule and let you know how it works for me. I'm having an upper endoscopy tomorrow! So hopefully I will have more answers about what is going on. Thanks

It's all about physics. If your body is on a horizontal position (ie. sleeping, watching tv, whatever) and ur stomach is not fully empty then acid can easily flow back. You need to empty ur stomach (3hours w/o eating/drinking), so that when you lay down, there's nothing left to flow back.

Some ppl recommend a walk after dinner, but i just dont have that habit/energy/drive. I do avoid laying down to watch tv though. It's the technique that worked best for me. Results were REALLY good. Let me know if it works for you as well.

This may be a silly question but when you guys say stop eating before bed does that also include just laying down? Sometimes I relax in the bed and watch TV before preparing for bed later.

I totally agree that doctors don't care!! I have been brushed off years!

Have you seen an ear nose and throat doctor? Mucus may cause the odor.  Also do you have regular dental cleanings done??  Good luck with your search

Hello Fedup1000,

I hope you find the root cause of your BB, but please understand that we all have different issues. Mine is related to a faulty valve (hiatus hernia) which allows digestive acids to flow back. To avoid taking pills to tighten that valve, I simply have my stomach empty by the time i go to bed, and that has solved my BB problem.

One thing I would add to what you've said is that doctors do NOT know it all, and are usually not interested in the investigation, therefore it's really up to each one of us to persist in 1) isolating the root cause of our problem (BB) and 2) finding the best way to address it, especially with all the information available on the internet these days.

Hey guys...here is some food for thought. I've spent a small fortune given this whole ordeal. Recently i talked to a few experts both in modern & alternative medicine and had an epiphany. What i realized was ....

"modern medicine essentially is designed to treat symptoms. This is how pharmaceutials company make so much money and this is what the modern doctor advocates. Even many of these doctors believe it themselves that they are HELPING you by helping you cure your symptoms. The reality is that we chase down one symptoms only to find another symptom to cure. WE NEVER ACTUALLY FIND THE ROOT CAUSE. Doctors usually say that THIS is the root causes, but actually it is another symptom. What i now believe is that we must change our thinking. The new paradigm of modern medicine is moving towards more natural ways of healing anyways. I do believe the human body is an amazing machine ... and it WANTS TO EFFICIENTLY CLEAN ITSELF and all we have to do is help it."

I now believe all these gastrointestinal problems that we have ... bad breath, gerd, whatever else you name it are all symptoms of a greater problem that modern medicine or doctor fail to recognize or overlook. What i truly believe that my ROOT CAUSE has to do with the LIVER. My doctors ruled that out, however i now think they are wrong simply because many doctors take it as a rule of thumb to dismiss it.

ROOT CAUSE: liver duct obstruction. This will effect your digestion, cause gerd, cleaning toxins, etc etc etc... many times difficult to diagnose by ultrasound in early stages. I suggest all of you do research on liver cleansing. I found a book called "the amazing liver cleanse" which i will try. Certain liver cleansing techniques can be found online as well.

I will let you'll know the results. Share comments if you have any.

I've known I had hiatus hernia (and therefore reflux) for many years.
I pretty much tried everything that was recommended to treat reflux, including: eliminate fattys from my diet, pills to tighten the 'ring', do not drink or smoke, pills to reduce acidity, incline my bed, etc. To some extent it helped to reduce gastrites/heartburn, but not BB.

About a month ago, i visited a few doctors (ear nose, gastro, allergist, family doc) and did a bunch of tests that came out inconclusive. Three of these doctors said I didnt have BB (I wish they were right and all the embarassing situations i've been through were just a dream). The allergist though, enphasized that I should stop eating 3hours before bed time, which I did and I immediatelly noticed a dramatic reduction of mucus on my throat and nose in the morning. I also noticed that people are not reacting to my breath anymore (covering / scratching noses, etc) which to me is an indication of improvement of my breath quality.

On a side note, I also use a mouth wash from Thera Breath helps to keep my mouth fresh for a good while, especially overnight.

To summarize, what i suggest:
1) Read about GERD and take the dietary suggestions to prevent it
2) Stop eating 3hours before bedtime
3) Incline your bed
4) Gargle with Thera Breath's mouthwash (others may work as well, up to you)

Would be nice to know if this works for you as well.

No H.pylori bacteria in the stomach was found.

Need some clarification: Fruitshakes are you saying that your hiatus hernia was the root cause of your killer bad breath? and not eating 3hours prior to sleep has eliminated the problem?

I suspected this problem ... however after endoscopy and x-ray doctors ruled that out. Can you provide me with more information on the results and diagnosis of your hiatus hernia.

As indicated before my tests for H.Pylori came negative. In any case i take probiotics (Acidophilus & Bifidus) daily.

You may have problems with bacteria in the gut.  Check out symptoms of (H. pylori bacteria).  Check out taking good bacteria such as probiotics.  This may reduce any bad bacterial buildup in you gut.  Good Luck!

Oh...thaks for clarifing that.....

No, my surgery was for chiari malformation.....basically it was brain surgery.
I had it May 28th and am still healing......all the meds and drugs used for the surgery affected my gerd.

3h = 3 hours
As mentioned before I stop eating 3 hours before bedtime. That is enough time to empty my stomach which means there is no reflux overnight. Because there is no reflux, there is no irritation of my throat and therefore my body doesnt release as much mucus as a reaction.

Another benefit is the fact that I dont have acids escaping from my stomach and damaging my upper digestive system.

HI...I already am doing all that except for the exercise.
I had surgery recently and  am still healing. I am just getting in a pool for some exercise.

I do not know what u mean by 3h??

can u please explain that.- this info on diet and lifestyle changes is posted in our Health Pages here on MedHelp, but if this is something that is not included, I would like to add it.

My diet is the traditional GERD diet:
I avoid fried/fat foods
I avoid citric juices (although I handle diluted frozen orange juice well)
I dont smoke
I dont drink (I do have a glass of wine if my stomach is not empty)
I also avoid eating chocolate at night.
I exercise a lot (7 times / week)
My bed is inclined (I piled 2 thick phone books on each side of my bed head)
If all fails, that still makes up for a very healthy lifestyle.


I had been doing all the above for the longest time, but what really made a difference was stop eating 3h before bedtime.

I am so glad u r getting results that apprear to be positive...a hiatal hernia can cause gerd and reflux, but it can be managed with diet and the elevated bed.....eating times..which u r addressing.


May I ask a bit more on this diet?...what is it?

Update:
Last month's tests indicated I do not suffer from:
- H.Pylori
- Sinusis (at least not enough to cause BB)
- Allergies (aside from the usual)

A recent endoscophy confirmed hyatus hernia which causes Reflux/GERD, so I focused on treating the stomach rather than my mouth/throat.

3 weeks ago I decided to stop eating anything 3 hours before bed time (SO HARD!) and I think it has helped to reduce BB. It's hard for me to know cause I have to rely on ppl's reactions to my breath, but I am looking for signs of improvement due to that change in my diet. It CERTAINLY reduced a lot of the mucus that would gather on my throat during the night, and that alone is a huge improvement. I dont have to keep clearing my throat as often now.

I also keep my bed inclined to help prevent reflux and ABOLISHED alcohol from my life. Im always the sober/boring guy at parties, but at least I dont release those foul gases anymore.  Oh well.

Do you guys wanna try the 3h/diet and share results?
