Possibilities of transmission of HIV & Other STD

I was done hiv antibody test. Initially my two hiv antibody test that was negative, 1st test was performed after 8 months & 2 nd test was performed after 10 month from 1 St test.
Now I do one more test for hiv i.e.hiv duo combo test after 2.2 year that is also negative.But at that time my blood collect by vaccutaioner and I don't know how vaccutaioner collect blood because 1 St time I gave blood by vaccutaioner And at time of my blood draw while attaching vaccutaioner needle to the holder the back sleeve of needle was removed I don't know because of technicians mishandling or something else.so he insert needle first in to my arm and after that the back sleeve was connected to the needle.
So my question is that technician connected needle to holder initially without back sleeve at that time it is possible to enter some blood from vaccutaioner holder enters into needle from back end from needle because I don't see vaccutaioner is clean or not?
Do I need another any hiv and std test.
Please suggest doctor.I am extremely worried about that.

I was just curious as it seems like you've posted here before and you should let us know if that is the case. A condom slipping is not a risk for HIV. The only risks are having unprotected vaginal or anal sex or sharing IV needles to inject drugs. As long as the head of your penis was covered, no risk. As to your discussion of testing methods, also no risk. INJECTIONS are different than a blood draw. So, no risk. No reason to test again about this or worry
Sir, I am asking that when needle was fitted to vaccutaioner holder the back sleeve of needle  was open so  some blood if present on vaccutaioner's threads that can enter into backside of needle while fitting needle to holder.Because he connect sleeve after fitting needle to hole.

Because that was hiv testing lab so for many people's the same holder can used before me.So I am worried.
Sir please need suggestion
The only risks for HIV in adults are:
1) Having unprotected anal or vaginal sex, or
2) sharing intravenous needles with IV drug users.

If you do not engage in any of the activities I mentioned above with someone HIV+ or whose HIV status is unknown to you, you will NEVER have to worry about getting HIV. There is no "what if" question that you could imagine about this event that would make it a risk for HIV.
