Best timing for mitral operation

Dear Doctors,
What is the most optimal timing of mitral valve surgery for a rheumatic mitral stinosis patient who encountered a cerebral emboli from left atrial thrombosis today?  There are still some thrombosis left in the atrium found by Echo.

Your welcome.  I can relate as I have moderate to severe mitral valve regurgitation. The MVR  has been going on for 7 years without any change in signs and symptoms, conditions can change very quickly, and I don't to be involved in an emergency situation regarding my MVR and any other heart disorder. It may be time for me to make a decision to go for an operation.

The best time is before there is any left ventricle enlargement and shortness of breath.  To wait too long, runs the risk of left ventricle dysfunction and increases the risk of surgical intervention as once the left ventricle has lost its ability to adequately pump enough blood/oxygen, it will almost never recover subsequent to surgery.
