Angiography report help

I need to understand the attached Angiography report,

Little history about the case:

57 Year old male, felt the chest pain & pressure, like some one is standing on chest. He has high blood pressure problem since few years now. Doctor suspect Angina initially, another doctor suspected the CABG after looking into the ECG and requested the Angiography.

Can some one read the report and let me know what can be diagnosed from this report.
