My result after a month take entecavir

My test
11/2015
qHbsag: 9645.50 iu/mL high reactive
AST: 323
ALT: 881
Then I take entecavir 0.5 mg daily. I also take chanca piedra as strong tea.
The results after a month:
qHbsag: 105.84 iu/mL reactive
AST: 28
ALT: 26
I want to cure. My doctor said me to continue with entecavir.
The first time of interferon at 2013 is terrible. Please advice me what can i do now.

attention i ve noticed you have confusion with naps and nucs

naps=replicor drug which can cure hbv in combo with peg or zadaxin

nucs= antivirals approved and used since 2000 on hbv

dont mix the terms they are totally different drugs

Unclear how naps would influence this situation, but likely he can get a high antibody if he uses them but a combo together with thymosin alpha is most likely needed.

in some patients this repeated flare pattern does finally lead to hbsag conversion. But the risk of rapid progression to cirrhosis is high and suppression of future strong flares by antivirals is of critical importance.

i'm appreciate for your answer. I still continue using env, beside my doctor advice me combined with A-HPC (Active Hexose Correlated Compound - AHHC 500mg) to boost my immune system.

when you have flares of hepatitis as humin does,  will naps keep the viral load and alt normal and keep inflammation down. will he ever svr because his hbsag quant is low or is this more dangerous than hbeag + and - chronic hep b.

This looks like intense repeated flares of hepatitis, without final hbsag svr. Some patients have this pattern and it is dangerous.

You are well advised to continue the entecavir  to supress a possible new flare in the near future.

A fibroscan should be done also.

The explanation for this pattern is not trivial.

I suspect the Chanca peidra for his hbsag reduction. Where you taking any other things beside entecavir & Chanca piedra? Thanks!

i guess entecavir is first line medicine among nucs. As far as I know tenofovir is not approved in Russia. I personally was on peg.

Why have you stopped interferon when HBsAg was so low at the end ? There was definatelly some response if hbv dna were kicked down to und.

Can you tell me what treatment are the good doctors recommending the most and which one the least, as the first line-medicine.
Basically, please rank interferon, entecavir and tenofovir from 1 to 3 according to the majority of recommendations of russian hepatologists as the first medication that they recommend in the treatment.

Here in Russia we have nucs/peg available, you can negotiate with your doctor many things so actually no problem pushing doctor for interferon if you want. By the way you do not need doctor prescription to buy interferon in drug store. Interferon is widely used here but it is significantly more expensive than nucs.

needadvicewithhep: he doesn't need to check anty-hbc igm its negative for sure, he was chronic in 2013, otherwise he would't get interferon

What was your HBsAg quant, hbv dna and HBeAg before and after interferon ?

Dear Sorte: This is my test before and after inteferon from 3/2014 - 3/2015, sorry my mistake not 2013.
* Before:
AST: 663
ALT: 1107
GGT: 380
HBEAG: 610.2 reactive
qHBV DNA: 1.46 x 10 ^6 copies/ml.
qHBSAG: not check.
* After:
AST: 28
ALT: 38
qHBSAG: 59.38 iu/ml
qHBV DNA: undetectable.

That's very strong response, congratulations :)
Don't worry if you hbv dna will be still detected in the next test, it's much harder to kick down HBsAg than hbv dna and your result is great keep going with etv and inform us :)

Hi Andrey...I was just today trying to find articles on how does Hep B gets nowadays usually treated in Russia but wasn't able to locate any good ones.

Could you please give me (us) the overview of whether the doctors are presenlty using interferon in current medical practice or is everyone treated almost exclusively with entecavir or maybe tenofovir....

Btw to be "functionally cured" means hbsag non reactive and hbsab reactive.

In short terminology "SVR"

HBSAG huge log reduction.

Looks like a natural immune response.

Igm anti hbc test needs  to be done (you might still be in immune clearance)

Check DNA and E antigen and I suspect they will be undetectable  also check for e anti body
the only concern would be the dramatic drop in ALT as it could signal immune tolerant phase.

Just my opinion im not a doctor

I believe you have a great chance of cure...and even if you don't get cured your hepatitis will most likely become inactive....

You need to measure your DNA, to see how entecavir is suppressing your virus.

As you probably know by know there is no cure other then the interferon (which can't cure most of us). There won't be cure within next 5 years unless Replicor is what they claim to be and some big company buys them soon and pushes their research....so just stay put taking your pills for now on...
I expect in 5-15 years from now we'll have something on the market that will be able to cure majority of us.
