help with result interpretation

hbsag II (patien'ts count 2,809) reactive; anti-hbs (2) negative; anti-hbc IgG (0.010) reactive; anti-hav igm (0.297) negative; hbeag (0.122) negative; anti-hbe (1.050) negative

I know I'm a chronic carrier and the doctor didn't advise any meds for me but I want a 3rd opinion or a 4th one if that's possible.  Thanks.

dont worry very rare and nothing to do with your state, using hbvdna pcr new tests (sensibility lowest detactable 6iu/ml) will show if you have it once hbsag is negative and hbsab pos

very lucky you are acute at the moment, make very healthy life and take good rest to avoid acute to ever become cronic

check ur vit D level and if it is not optimal, less 50ng/ml, take supplement.vitamin d is an hormone which regulates immune response, white blood cells don t get activated if there is no vit D

you re immune from a long time so you lost hbsab antibodies, it happens sometimes and also a good sign of complete hbv eradication
your immunity is ok even without hbsab for future infections but if you want to be 100% sure  just make a booster dose of vaccine to gain hbsab again since it is easy to meet it again if nigeria has no hbv vaccines programs at birth

widely available everywhere except US, canada and UK

if alt normal or just boreder line accuracy can be higher than biopsy to exclude relevant fibrosis, not good to detect f1 fibrosis but we dont care about f1 fibrosis since it is not relevant at all and can also be due to sedentary life/fat foods

france, germany and italy use fibroscan routinely from many years (it is available since 2004) and biopsy is not used anymore except particular cases or if it is necessary to detect f1 fibrosis

I'm ruling out a biopsy since I already had an ocular observation from my obygyne during one c-section many years ago.  I had normal alt/ast back then as part of routine prenatal for being hbsag positive.  Had I known back then....I should have gone for a biopsy.  It's quite daunting to go under the knife again for a biopsy.  Any more information regarding the fibroscan? Appreciate your thoughts.  Thanks.

alt/ast can be normal while you dye of liver failure and are usually normal on cirrhosis.5 years ago we thought they can be a sign of liver damage/disfunction but we now know it is not so, although on fewer percentages liver dmage is found also in those with normal alt/ast, liver function blood tests

are there any other external signs to indicate liver damage?

no, i had all normal blood tests with nodular cirrhosis and still have.you feel perfectly healthy even months from death for both cirrhosis and liver cancer.

cirrhosis signs are low platlets, low inr, higher bilirubin but when you have these signs cirrhosis cannot be regressed and it is so advanced that might be deadly.
since we have both biopsy and fibroscan available it is extremely stupid to wait and see if blood tests get abnormal

i am very fit, sports minded, no alcohol intake and non-smoking
and it is possible to have cirrhosis or liver cancer while you feel so good if you never checked, only those tests will tell you 100% how your liver is

this test is useless for cronic patients, you have to make abbott architect hbsag quantification or roches hbsag quant.the results are expressed as iu/ml with lower range sensibility of 0.05iu/ml.all other types of test without iu/ml number are uselss

anti-hav igm (0.297) negative
hbv is cronic

hbeag (0.122) negative; anti-hbe (1.050) negative
antihbe is better positive, alinia (nitazoxanide) or interferon can make this positive.now you are in a balance between hbeag and antihbe

you are missing the most important basic tests to know what your conditions is:
hbvdna pcr
fibroscan or biopsy which are the only test to detect liver damage, all the rest can t, if we dont know if liver has damage or not we cannot even talk about a therapy
alt/ast

alinia (nitazoxanide) or interferon are the only safe immune modulatory drugs that have effect on hbv.all the other antivirals have no effect on virus and can only block liver damage in case there is any
