Next steps for dx and tx?

I have had a kidney stone and infection in the past.  For the last two weeks I have had rt side back/flank tenderness and intermittent pain below ribs and in groin area.  I went to the physician's assistant since my MD was out of town and she just told me to drink more water.  I have been hydrating.  I have waves of fatigue and nausea (not severe) and have been lethargic this entire time.  I am concerned that I am not passing what I am consuming.  I have put on 10#s since I weighed a week ago without having eaten anything that would have accounted for the weight gain.  My hands and feet do not seem to be swollen. I also have some discomfort when urinating.   What now.  Do I need to get a CT scan to see if the stones are not moving???? Do I just wait it out????  Do I need to check for UTI?????

Hi, although drugs like ciprofloxacin 500mg twice daily is usually recommended for urinary tract infection, but one should go by urine culture sensitivity report. For pain associated with infection drugs like ibuprofen, diclofenac etc are prescribed as they have potent anti inflammatory action.

Please follow doctor’s advice and avoid self medication.

Thank you for your response.  Yesterday, when my regular MD returned, I saw her and she ordered blood tests (presumably for Kidney function), urinalysis (to assess infection) and a CT scan.  I find out the results on Thursday.

Regarding you recommendation of antibiotic therapy, I am assuming I wait for the urinalysis results....   Also I am not familiar with the use of anti inflammatory drugs.  Could you elaborate???

Hi, get an ultrasound done to rule out obstruction along with urine examination for infection. In case ultrasound results are not suggestive for stone, other possibilities like renal compromise etc should be investigated.

Start antibiotic therapy along with anti inflammatory drugs.
