You seem to be going through your early puberty. All these problems will continue for a few more years, till you get control on your emotions. In the mean time spend more time in creative activity and take quality food and do moderate exercise; these years are very critical for your body and mental growth.

Stop sexual activity completely. To stop it learn abdominal breathing. (Google it). Practice this breathing for 10 minutes twice a day. When you are sexually excited, use this breathing to calm your self. In 2 or 3 months you will regain your sexual drive. Take care of your general health.
