When did they first check her hCG levels? More than a day ago? If it seems to be an emergency, not waiting 48 full hours would be acceptable; they could get the information they needed in 40 hours, say. (They are looking at a hormone produced by women who are pregnant, and if it doubles in 48 - 72 hours, that is considered to be a good sign for the pregnancy to be progressing normally.)
However, if she is only seeing blood when she wipes, it is not an emergency at this point, and it would be a good idea to avoid getting the flu at the ER if you can help it. She should lie down -- basically stay off her feet, to calm uterine contractions. Only get up to pee and eat. She should stay hydrated. (Dehydration can cause the uterus to start contracting.) If things calm down, then it was just some uterine bleeding, and she can get up after she is no longer seeing bleeding (my doc said 24 hours after the bleeding stops.) There are other reasons besides miscarriage that a woman can have bleeding in the uterus in pregnancy (a broken blood vessel, etc.)
IF she has any of the following, call her doctor for advice or go to the ER.
- Bleeding that is enough to fill more than one maxi pad in an hour, or "flooding" by the cereal-bowlful when she gets up to go pee. (Just blood when wiping or urinating is not this level of problem, but she will want to watch for things getting worse.)
- Faintness
- Fever
Please also remember and reassure her, that miscarriage (especially early miscarriage) happens for the embryo's own reasons. You can lie down and drink water all you want and lose a pregnancy, or go hang-gliding and hop on a pogo stick and keep a pregnancy; it all depends on whether the myriad millions of connections that have to be made as the embryo develops, all connect right. If something does not, the embryo will grow until that thing is needed, and then it will fade because that thing is not there. Not the mommy's fault, nothing she did or didn't do. The bed rest and fluids are merely to help if something (other than the embryo) is problematic, such as uterine contractions because of dehydration or a small popped blood vessel.
