custom orthotics don't work

First a history. I am in my mid-30s. I have been wearing custom orthotics from a podiatrist for 15 years. For about the first 6-7 years I wore the same orthotics. They were hard. They worked. I could only wear them for about 3-4 hours at a time with out pain. So I would take them out. But they shaped my foot enough to function. I used to be on my feet all day for work.

By about early 2000s I started seeing limitations on how much I could be on my feet. I was getting treated for back pain by a doctor (not a foot doctor) and he recommended I try over the counter Sole insoles that were softer. These worked great for a few years. Then 2 years ago. Everything started to hurt. Legs, feet, soreness in the morning, calves.

Went back to a podiatrist (new one). I got x-rayed and new orthotics. He said it was normal to need to change orthotics. These new ones were higher and harder than the sole, but softer than my original ones. They worked for a year. Then I had alot of pain.

Went back to the same podiatrist and got news. These have been adjusted repeatedly over the last year and no matter what I do my feet hurt. I am getting new ones made that are softer. Right now I am wearing a Dr. Scholes insole over the orthotics. As recommended by my doctor.

I also have small bunions on both feet. However, I have really wide feet. So I am wear New Balance 6E shoes. He said I will need bunion surgery eventually.

I got molded for the new orthotics last week. My old ones are getting worse on my feet.
1. I have pain in the bottom of my feet all day. I have had these for about 3 months and been wearing the Dr. Scholes inserts for 6 weeks. This is new. I am not on my desk alot
2. I get pain my toes in both feet. I get pain in the right toe. It is not the same as bunion pain. Its like I am putting more pressure on my toes. He has me prescription Naproxin. I am getting worse.

A few weeks ago I could manage 1.5 hours on my feet at a time. I had pain 20-30 minutes in but it was mild. Now I get pain just sitting at my desk. I have soreness in the morning even though I ice my feet at night.

I got alot of I don't knows from him on my last visit. We took x-rays again and he said my feet are badly flat footed. I asked him if I should go to an orthopedic surgeon yet for a second opinion. He said no lets see how the new softer orthotics work. My feet were not this sore 2 weeks ago. I am waiting for new orthotics, but I am  not convinced they will work and the pain gets worse every day.
