ultrasound results and very uncomfortable

I am 33 and have 2 children, 7 and 3.  I have been experiencing hip pain off and on since thanksgiving.  I thought it was from walking alot in flipflops.  9 days ago, I had no appetite at all during the day and then thought I had a stomach bug that night.  Severe abdominal cramping and diarhea for about 5 hours.  Since then my hip pain has been constant, running all the way down my leg on right side. 6 out of 10 for pain.  Also I have been having menstual type cramps for the last 7 days.  I get bloated everytime I eat or drink anything and that causes the pain to get worse.

I went to dr for the hip pain and she said bursitis, but sent me for an ultrasound due to painful periods and extreme pain during and after sex.  The transvaginal ultrasound magnafied my hip pain by 20 times but only when she was pointed at right ovary.  I would say it was a 9 out of 10, with 10 being natural childbirth.

As far as other history goes, when I was in my teens and early 20's I would get very bad periods and cramping that was unbearable.  Sometimes I would lay on the floor in a ball crying from the pain.  I also had times where i would be in so much pain from sex that I would throw up.  Once I went to the hospital and they said it was gas.  I haven't had anything like this again until recently, however for the last 8 years I have been either pregnant, nursing or had iud so no periods.  Had to go off IUD due to complex migraines mimicking stroke symptoms but that's a whole other story.

Here are the US results:
The anterior left wall of the cervix is 2.0 x 1.4 x 1.3 cm hyperechoic vascular mass.  This does not definitavely extend into the cervical canal.

The right ovary measures 3.7 x 2.6 x 2.9 cm for a volume of 15 cc and contains a collapsed 2.2 cm hemorrhagic corpus luteum cyst.  Scanning over this region, the sonographer reported significant tenderness.  Small to moderate amount of free fluid in the right pelvis and superior to the uterus.

The left ovary appears normal measuring 3.8 x 2.0 x 2.0 for a volume of 8cc.

Impression:
1. Right lower quadrant pain likely cause by a hemorrhagic right corpus luteum 2.2 cm cyst.  Small to moderate amount of free fluid in the pelvis suggest partial rupture.  Patient did have significant tenderness during scanning over this region.

2. 2 cm echogenic mass in the anterior left wall of the cervix.  This is indeterminate and may represent a cervical fibroid or other cervical mass.  This does not appear to be a cervical polyp.  Recommend gynecologic consultation and physical evaluation with possible ultrasound follow-up.

So here are my questions.  If the pain was due to a ruptured cyst, shouldn't it be getting better by now, not worse.  Other than obviously being something the US echoes off of, what does hyperechoic and echogenic mean/indicate?  Should I push for laparascopy to look for endometriosis/cervical cancer/ovarian cancer?  Do I just have to wait out the pain?  I have an appointment with the university women's clinic on Tuesday, but like everyone else, I imagine, am having a hard time being patient when I am in such pain.  Thanks for your time.

I'm so sorry you're having this kind of pain and I understand your worry.. But....first of all, let's not jump to any conclusions.. If , indeed you had a cyst rupture, it can take awhile to feel better. Second, cysts are really very common with premenopausal women and 95% benign. Third... you really need for a Dr. to interpret these results for you.And finally, it's good that you have an appt. Tuesday. Take all these questions (written down so you don't forget them) and insist on some answers..

In the meantime, why not post the question regarding the cervical "mass" on the women's health forum as well. You may got more answers there regarding that issue. And... try not to worry. If, indeed, a laparoscopy is in order, there are lots of us that have "been there, done that".. It's not really much to worry about and is usually out patient. but please, don't jump on the worst case scenario .. Check the top right of this page "Health Pages" and read more about cysts..

Good luck and keep us posted,
Sandy
