Although I'm not a doctor, I do think that you've been extremely diligent in regard to following up with STD testing and doctor visits, which is excellent. I understand your concern, but I think you should feel more reassured at this point about being clean. If you still feel uncomfortable, use condoms during any sexual activity.
In regard to your split stream and weak urine; maybe have your doctor (or Urologist) take a closer look at your urethra.

The biggest concern I guess I have is I am starting a new relationship with an awesome woman and I don't want to potentially give anything to her. She is clean and with all of my testing I would assume that I am as well. That is the core concern of mine.
Here are all of the testing that I did.
4 weeks post encounter: Negative for all of the following: Gonorrhea, Chlamydia, Syphillis, HSV1/2 (IGG), HIV 1/2 (4th gen test), Heps A/B/C.
7 weeks post encounter: Negtive for all the following: Gonorrhea, Chlamydia, Syphillis, HSV 1/2 (IGG), HIV 1/2.
Urine culture healthy. Urine Dip Test healthy.
14 weeks post encounter: Negative for all the following: HSV 1/2 (IGG), Syphillis.
16 weeks post encounter: Negative for all the following: Gonorrhea/Chlamydia.
Urine Cuture healthy.
21 weeks post encounter: Negative for the following: HSV 1/2 (IGG), Syphillis, HIV 1/2 (4th gen test).
During my initial rounds of testing I visited two dermatologists and one urologist. They looked at everything and thought all was fine. So I assumed everything was okay. It could be due to hypervigilance. Not sure on that. I do experience split stream urination now as well. When my stream is week, it starts out as two streams and then merged into one, this seems to happen way more frequently.
With that I have multiple negative STD tests. So I would assume this would not be driven by any STD. Also two urine cultures came back healthy as did a urine dip test. I am a male who just turned 40. Is this normal? Does anything sound like something I should really be worried about?

I'm sorry you haven't received any replies yet; hopefully I can be of some help.
I suggest researching thrush, or male yeast infection; you can do this online, to see if your symptoms match the description of what you have. Sometimes there are actual pictures, along with the descriptions that can be more helpful... If this is something that continues to bother you, you should seek advice & treatment from your doctor soon.
