Tell the doctor or nurse that this is your first pelvic exam, and ask him or her to explain everything.  Only an idiot would fail you in those circumstances.  Doctors can be very informative if you are interested.

Or, do what I did my first ob exam.  While you're climbing onto the table in your gown and draping yourself with the paper drape and waiting, envisualize yourself leaping up the second the doctor walks in and screaming and running down the hall clutching the drape around you.  Imagine his or her astonished face (plus the face of the nurse out in the waiting area).  This will probably make you giggle to yourself, a relaxing thing at such a moment.

You can expect to be asked about your past health history including, first time you got your period, how long it lasts, last period, are you birth control, are you sexually active, sexual practices (condoms)
They will also take your vital signs, like blood pressure, weight, pulse etc. They will ask you to undress completely naked and you will get a stylish (being sarcastic) exam room gown to wear.
When the doctor comes in, he/she will speak to you about your history, why you are here and hopefully explain to you what will happen that day. It may start off with a neck and breast exam. You will be asked to lie back and the doc will press your stomach.
The fun will continue with you being asked to place your legs in the stirrups, you will have to scooch down so that your bottom is at the edge of table. Doc will tell you to relax and try to. Hopefully a warm or room temperature speculum that is lubricated will be inserted into your vaginal opening. It will stretch. You may feel a bit of discomfort, especially when the pap smear (and if you get std testing)
After that the doc will place two fingers into your vagina and gently press your lower abdomen.
Then you are done!

I have been going for a long time. Personally, i feel it's never comfortable laying spread eagle for the doc to see but it helps because 1) sometimes i have issues that i dont know about that the doc notices and 2) cancer runs in my family so I go to make sure I am healthy. I find that the edge of being exposed is lessened when I rattle on about nothing. I swear, i talk about the weather and the Mets--even though I don't watch baseball :) It helps.

No worries, you will be fine. You'll survive.
