Ovarian cyst with mural nodule by ultrasound

My wife, 37, did an pelvic ultrasound last week because of irregularity of her recent menstrual cycles.  Last night, she was extremely worried when she showed her ultrasound image CD to a friend who is an ultrasound specialist because the way the friend wanted her to be "prepared and strong".

Her doctor did mention surgery in the post ultrasound visit on Saturday.  (Can't be sure if it was laparoscopy or laparotomy.)  However, in the ultrasound report, attached at the end of this post, he seems to indicate to wait for 4-6 weeks of waiting.

Our concerns:

1. Should we wait?
2. What are the best and worst scenarios? Odd?

Also, at the end of the report it says "no free fluid is found", is it good or bad?

Any pointers/suggestions will be highly appreciated...

Pelvic Ultrasound:

Clinical History: Evaluate for fibroids.

Findings:

Real time ultrasound both endovaginal and transabdominal were utilized to evaluate the pelvis.  The uterus measures 9.1 x 4.6 x 5.5 cm.  There are two fibroids anterior right measuring 1.8 x 1.7 x 1.2 cm, left 1.8 x 1.6 x 1.7 cm.  There is a retroverted uterus. The endometruim measures 8.6 mm.  The right ovary measures 7.3 x 4.6 x 5.5 cm.  The left ovary measures 2.9 x 1.5 x 2.2 cm.  There is a right ovarian simple cyst measuring 5.7 x 4.5 x 5.1 cm.  Size of this is of concern and there does appear to be a questionable small mural nodule.  Follow up in 4-6 weeks is essential   If present at that time clinical correction to exclude cystadenoma, cystadenocarcinomas is recommended.

Impression:

Prominent right ovarian cyst as described.  Follow up in 4-6 weeks is essential to exclude underlying malignancy.  This cyst is predominantly simple, however, there does appear to be a small possible mural nodule within this cyst.

No free fluid is seen

If it were me, I would get an appointment with her gynecologist and tell them you want to know exactly what the results mean.  It sounds like she has a complex cyst which has both solid and fluid within the cyst.  No fluid could mean that there is no fluid in the cul-de-sac which is a good sign.  Please keep us updated on how she is doing.  DON'T LET THIS BE, MAKE SURE TO FOLLOW UP WITH HER DOCTOR!!  We have to be advocates for ourselves, no one else will do it for us!!

hi dont know much yet to advise all i say you will get great advice and support on this forum that know what their talking about all i can say is keep posted i wish you and your family well .

Take these results serious and follow up per doctors orders, anytime their is a cyst that is solid, meaning no free fluid you have to take it serious.
