It's hard to say what's happening because you haven't actually been tested for anything. That's a shame.
From receiving oral sex, you can get gonorrhea or chlamydia, though gonorrhea is more likely. You could also get NGU, which is an infection in the urethra.
Doxy and levfloaxcin would cure chlamydia and NGU, but not gonorrhea. Now that you've been on doxy, it may give you false negatives on chlamydia if your antibiotics failed.

Gonorrhea is more likely with oral sex than chlamydia, but both are possible.
2) How likely is it that I’ve passed either of these onto my girlfriend?
If you had either, or NGU, it's pretty likely.

Neither would cure gonorrhea.

Gonorrhea might. You'd probably get a false negative for chlamydia. Test anyway and see what it says.

Not if you had gonorrhea.

Yes, because you could have gonorrhea.

This is something you should ask a pharmacist. The alcohol probably wouldn't cause a failure, but I don't know about the calcium.
