Hashi Moto thyroid disease

My son has extreme fatigue and is sleeping a lot.  We have ruled out any depression.  He has a rare gastritis disease which caused anemia.  He also has hashi moto.
Labs
TPO - 416
Thyroglobulin - 8.9
T4 - 1.48
TSH - 5.260
His synthoid dose was bumped up after these labs.  Could hashi moto still be causing severe fatigue or should we look elsewhere?  He is sleeping a lot and still doesn't look any better.  He gets viruses/colds often too.

What was his Synthroid dosage before and after those labs?  Also, what is the reference range shown on the lab result for the T4?

Hashi's itself doesn't cause fatigue.  The fatigue would be due to inadequate thyroid hormone levels and/or other important factors like Vitamin D, B12 and ferritin.  You should make sure to get him tested for both Free T4 and Free T3 every time he goes for tests.  Also, if not tested for Vitamin D, B12 and ferritin those should be tested also because hypo patients are frequently too low in the ranges for those.  D should be about 55, B12 in the very upper end of the range and ferritin should be about 70 min.
